Core Values
CT InCK serves eligible children, youth, and families in New Haven, Connecticut, including:
Consistent with the program’s whole-family approach, while eligibility is determined by the enrolled child or pregnant/postpartum individual, care coordination extends to support the surrounding family and caregivers.
Families may be referred by a provider, identified through community outreach, or self-refer to the program.
A comprehensive assessment identifies each family's strengths, needs, and areas requiring support.
Each family is assigned an Intensive Care Coordinator (ICC), who serves as a consistent point of contact responsible for organizing care, reducing barriers, and ensuring follow-through across services.
Services are provided in-home, in the community, or virtually, based on family preference and need.
